• Release of Medical Records to Grandville Pediatrics (to allow transfer of medical records from another medical office to Grandville Pediatrics)
• Release of Medical Records from Grandville Pediatrics (to allow transfer of medical records to another medical office from Grandville Pediatrics)
• Patient Registration Form(contact and insurance information)
• Initial History Questionnaire(for all new patients- gives information on past medical, surgical, family and social histories)
• Receipt of Notice of Privacy Practices (form that states that you have received a copy of HIPAA and privacy practices form)
• Authorization for Medical Services (form that allows another caregiver to bring a child into the office and receive medical care- needed for all children under 18 yo)
• School Health Appraisal Form (form for school entry and daycare)
• Sports Physical Form  (form for students playing high school sports)
• Patient Authorization Initial Form
• Financial Agreement between 18yr patient parent
